Flu Shots

Attention Employees:

We’re excited to announce that we’ve arranged to have flu shots offered to Prosper employees! It’s free for employees, their spouses and their children 18 years and up who are enrolled on the SelectHealth plan.  There’s a cost of $25.00 for those who are not enrolled on the SelectHealth insurance.  If you have other insurance, you can bring your insurance card and CNS will bill your other insurance for the flu shot(s).  Please bring your insurance cards on the day of the shots even if you are covered under SelectHealth. 

We have scheduled the flu shots for the following dates:

Provo location:  Wednesday 10/26 from 11 a.m. to 1 p.m.

HR Training Room.

Draper location: Thursday, 10/27 from 11 a.m. to 12 noon.  Draper Conference Room.

Rexburg employees can go to Walgreens for their flu shots.  Walgreens will accept the Nationcare insurance.

Please RSVP to Susan Young in Human Resources by Monday, October 17^th to reserve your spot.  (syoung@prospering.com).
